Accessing and using the Application Forms

Applying for prescription of a new qualification not previously prescribed by ARB

• Please download the New Qualification Application Form and save it on to a disk/computer hard drive. Please refer to the Guidelines for completing the ARB Application for Prescription of a New Qualification Form for further details on completing each section of this form.

Applying for prescription of a qualification previously prescribed by ARB

You should refer to the Guidelines for completing the ARB Application for the Renewal of Prescription of a Qualification/s Form for further details on completing each section of this form

The Application Forms have been designed in Microsoft Word format to allow for the spaces and boxes to be completed in a typed format. Spaces within the boxes provided will expand as necessary.

Once you have completed the relevant Application Form, please print off a copy and ensure that you have signed the form in the relevant sections. You must send us the original, signed version of the Application Form with your full application.
